BOARDING RESERVATION REQUEST FORM

A $30 non-refundable deposit is required to secure all boarding reservations

**Credit Cards are only kept on file at owner's request**

Only returning customers may submit online requests.
New boarding clients need to call the office to make their first reservations.

To request a boarding reservation online, you must complete the following steps.

First step: Read our boarding policies page and check the required form fields below (* indicates a required field).

Second step: Fill out this form and click the Submit button.

Third step: Wait for a Boarding Confirmation call or email (You should receive one within 24 hours Monday through Friday).

Fourth step: After receiving Boarding Confirmation and your pet does not require medication/supplements/vitamins, etc., you may fill out a boarding check-in form to bring with you at check-in.

The boarding reservation process is not complete until you have received a confirmation call or email from a staff member, and you have secured your pet's reservation with the required deposit. Please contact a Town & Country Team Member if you have any questions about this process (919-387-7833).

Vaccination Requirement: I understand that in order for my pet to board, he/she must be current on his/her vaccinations. Dogs must be current on Rabies, Distemper, Parvo and Kennel Cough (required every 6 months.) Cats must be current on Rabies and Feline Distemper. If my pet is not current I understand that he/she will be vaccinated at drop-off by Town & Country Veterinary Hospital at my expense.

Flea Requirement: I further understand that in order for my pet to board, if fleas are found they will be treated with one dose of flea treatment upon arrival and will be bathed at owner’s expense.

Medications: It is very important that we are aware of any medication your pet will need while staying with us. All meds must be kept in original pill vial with the current directions on the label. Pets requiring medications / supplements / vitamins, etc., must use the duplicating Boarding Check-in form from our office.

Diets: We accept outside food for boarding pets at an additional charge of $1.00/meal. These meals must be in individual Baggies labeled with pet’s name, date and AM or PM meal. Due to storage limits we will not accept large bags, buckets, Tupperware, etc. of food. We provide a variety of holistic diets from the Pet Pantry. Unless our pet requires a prescription food or has a sensitive stomach we ask you to allow us to feed our food.
